The Partnership Promise

If you choose the Partnership PPO, you must commit to the Partnership Promise. Your incentive and rewards for doing so are:

• Lower monthly premiums
• A lower annual deductible
• Lower pharmacy co-pays and co-insurance for all types of medical care
• A lower out-of-pocket maximum

2012 Partnership Promise

Members who enroll in the Partnership PPO must agree to the terms of the Partnership Promise each year. The 2011 Partnership Promise encouraged members to become more aware of their health by completing an online questionnaire and by having a biometric screening. Members will not have to attend a screening or complete a health questionnaire in 2012. However, we are asking members to take a more active role in managing their health and wellness. The requirements of the Partnership Promise in 2012 are still simple.

This year, members and covered spouses must do two things:

1. Participate in health coaching if an opportunity to improve your health is identified by the Partners for Health wellness staff during 2012
2. Keep address, phone number and email, if you have one, current with your employer

Members with certain health conditions or risk behaviors will be required to work with a health coach. Together, members and coaches will set goals for improvement.

Members will also be required to keep their contact information up to date with their employers. If a member’s information changes during the year, it is the member’s responsibility to make sure the employer has the correct information on record. An email address is not required, but you MUST keep your phone number and mailing address up to date.

Both you and your covered spouse must meet the 2012 Partnership Promise requirements to remain eligible for the Partnership PPO in 2013. All members enrolled in the Partnership PPO must fulfill the Partnership Promise even if the Partnership PPO is not the primary insurance plan. Children do not have to meet the requirements of the Partnership Promise.

Updating Your Personal Information

Depending on where you work, there are a couple of ways to update your contact information during the year.

State employees:

• Change your contact information yourself in Edison
• Contact your agency's human resources office

Higher Education, Local Education and Local Government employees:

• Change your contact information yourself in Edison
• Contact your agency benefits coordinator
• Call the Benefits Administration service center at 1.800.253.9981 and select option 6
